Which starting option should I choose?
Choose a guidance appointment if you would like to discuss your form requirements first. Choose Standard or Priority Review if you are ready to submit your form and supporting information. All three options book directly through Jane. After booking, receive and complete your Medical Form Intake and securely upload your documents. You only need to choose one starting option.
Is every medical form privately billable?
No. This service is for eligible uninsured work. Some forms or related services are covered by a public plan or another funding program. Eligibility, applicable funding and the physician’s professional requirements must be checked before privately paid work proceeds. For appointments, eligibility also depends on your physical location.
Do I need to have seen this physician before?
No. However, complete medical records and supporting documentation make it easier for the physician to determine whether the form can be completed without another appointment.
Will I need an appointment?
Not always. Some forms can be completed from sufficient existing information. You may choose a guidance appointment first, or a separate assessment may be required after document review. Any additional work and fees are explained before you agree to proceed.
What if the physician cannot sign my form?
The physician will explain the reason or identify what additional evidence or assessment is required. Payment purchases professional review; it does not purchase a guaranteed signature or outcome.
Is Priority review the same as guaranteed completion in five hours?
No. Most complete Priority requests received on business days are reviewed within five business hours. If the form is eligible and sufficiently supported, it is usually returned within 24 business hours. Complex forms can take longer.
How are my documents returned?
Completed documents are returned through the secure process associated with your intake. You can then download and forward them to the organization that requested them.
Can an employer or law firm submit a request?
Yes. We provide separate organizational pathways for eligible documentation, with appropriate consent and physician independence.
Can I request a medical form online from New Brunswick or Nova Scotia?
Eligible clients in New Brunswick and Nova Scotia can begin a request through our secure online process. A physician determines whether the form can be assessed using available records or whether a virtual or in-person consultation is needed. Availability depends on the type of form and applicable professional requirements.
Can a physician review a Disability Tax Credit (T2201) form?
A physician may review the medical information relevant to a T2201 request and determine whether they can certify the applicable sections. The Canada Revenue Agency, not Form5 or the physician, determines tax-credit eligibility.
Who makes the clinical decision?
Participating licensed physicians make all clinical decisions independently. Form5 coordinates intake, scheduling and administration; submitting a request or paying a review fee does not guarantee a completed or signed form.
How is my personal health information handled?
After booking, receive and complete your Medical Form Intake through the secure patient portal. Upload your medical form and supporting documents there rather than by public messages or ordinary email. Review the privacy and consent information provided during intake.
