Step 2 - Assigned
Physician Review Begins
Your request will be assigned to a licensed Canadian physician for review. The physician will carefully assess your submitted documentation to determine whether it satisfies the medical and documentation requirements for the requested form.
If applicable, your credit card will be charged any outstanding Professional Review Fee associated with the specific form requested.
If the physician determines that the requirements have been met, the form will be completed, signed where appropriate, and securely returned to you.
Each service includes a Professional Review Fee, which covers:
- Administrative processing of your request.
- Creation and management of your secure patient portal.
- Physician review of your submitted documentation.
- Completion of the requested medical form or documentation, where clinically appropriate and supported by the information provided.
The Professional Review Fee does not include additional virtual or in-person medical consultations or examinations. If the physician determines that additional assessment is required to safely or appropriately complete the requested documentation, a separate consultation may be recommended and, if accepted, will be billed separately.
Refund Policy
Once your request has been submitted the Professional Review Fee is non-refundable, regardless of whether:
- Additional medical information or supporting documentation is requested.
- A virtual or in-person consultation or physical examination is required.
- The physician determines that the requested form cannot be completed based on the available clinical information.
- The physician determines that the requested documentation is not medically appropriate.
- The physician recommends an alternative service or assessment that is more appropriate to your circumstances.
All medical decisions, including whether a form can be completed, are made solely at the independent professional discretion of the reviewing physician.

Can any medical form be completed this way?
Our service is optimized for Level 1 and Level 2 documentation. Complex Level 3 forms or those requiring extensive physical examination typically need an in-person appointment.
Do I need to have seen the doctor before?
No. However, providing detailed medical history and supporting documentation dramatically improves the likelihood of completion without requiring a new visit.
What if the physician needs more information?
We will contact you promptly. In some cases, a short virtual consultation may be required (additional fee may apply).
Is my medical data secure?
Yes. We use 256-bit SSL encryption for all uploads and storage. Your documents are accessible only to the reviewing physician team. The lead doctor is the custodian of medical records.
What is the refund policy?
Review fees are non-refundable. Participating physicians are professionally and ethically responsible for ensuring that every medical document accurately reflects the available clinical information and complies with accepted medical standards.
Each request is individually assessed by the reviewing physician.
Following review, the physician may determine that:
- the requested form can be completed based on the information provided;
- additional supporting documentation is required;
- a phone call, a virtual consultation or physical examination is necessary before the form can be completed; or
- the requested documentation cannot be completed because there is insufficient clinical information or the request cannot be supported medically.
Submission of a request does not guarantee that a physician will complete or sign the requested form. Physicians may only complete medical documentation when they are satisfied that it is clinically appropriate to do so.
Professional Review Fee
Each service includes a Professional Review Fee, which covers:
- administrative processing of your request;
- creation of your secure patient portal;
- physician review of your request; and
- completion of the requested medical documentation where clinically appropriate. Additional virtual or in-person consultations are not included and may be required at the physician's sole discretion.
Once your request has been submitted and physician review has commenced, the Professional Review Fee is non-refundable, regardless of whether:
- additional documentation is requested;
- a virtual or in-person medical consultation or examination is required;
- the physician determines the requested form cannot be completed; or
- the physician determines that a different service is more appropriate.
If a virtual consultation or examination is required before the requested documentation can be completed, any additional fees or billing arrangements will be explained and must be accepted before those services are provided. Exception: If PEIDoctor.com is unable to assign your request to a physician for professional review, your Professional Review & Completion Fee will be refunded in full.
Additional Fee Transparency: Physicians make all medical decisions independently of PEIDoctor and EndWell Management Inc. Your Professional Review Fee includes one telephone call, i.e. if the physician determines it is necessary. If the physician needs additional information that cannot be obtained by phone, they will usually arrange a convenient virtual appointment. Simple virtual consultations typically start from $59.
If an in-person consultation is required, you will be advised of the reason and provided with an estimated cost before booking. Providing clear, accurate information and complete supporting documentation helps the physician review your request efficiently and minimizes the need for appointments, saving both time and cost.
